  • a.CTでは病変の存在,大きさ,広がり,正常構造との関係を正確に捉えることができ,また無気肺や閉塞性肺炎の有無,隣接臓器への浸潤の診断にも有用であり,病期診断において侵襲的検査の前に最初に行うべき基本的な検査である。TNM分類第8版においてT因子は従来より細分化されているが,本邦においても胸部造影CTに高分解能CTを併用することでT因子の正確な診断が可能との報告がある1)

  • b.CT単独ではT3およびT4の診断が困難な場合があり,T3診断の感度が38〜90%,特異度が40〜90%2)3),T4診断の感度が40〜84%,特異度が57〜94%4)と報告にばらつきがある。一方,FDG-PET/CTは縦隔浸潤,胸壁浸潤,腫瘍周囲の無気肺との鑑別に有用5)で,T因子の正診率が82%と,FDG-PET単独の55%,CT単独の68%と比べて高い6)と報告されており,特にT3およびT4の診断には推奨される。

  • c.MRIも縦隔浸潤,胸壁浸潤,腫瘍周囲の無気肺との鑑別に有用とされ,T3の診断率がCTの60%に対してMRIが80%,T4の診断率がCTの33.3%に対してMRIが100%,無気肺の診断率がCTの61.5%に対しMRIが84.6%との報告7)もあり,胸壁浸潤,腫瘍周囲の無気肺との鑑別が困難な症例には考慮してもよい。ただし,GGNやT1およびT2の診断に関しては,FDG-PET/CTやMRIよりもCTのほうが診断率が高く7)8),FDG-PET/CT,MRIがCTに代用されるものではない。

  • a.胸部CTは短径1cm以上のリンパ節を転移陽性と診断することが多く,メタアナリシスによると,感度は59%,特異度は78%と報告されている9)

     FDG-PET/CTの切除可能な非小細胞肺癌におけるN因子診断に関するCochrane Libraryのシステマティックレビューによると,原発巣などの背景臓器における集積を基準としてN因子を判定した場合,その感度は77.4%,特異度は90.1%とされる10)

  • b.非小細胞肺癌の縦隔病期診断におけるMRI拡散強調画像をFDG-PET/CTと比べた43研究のメタアナリシスによると,10研究から解析した前者の感度は72%,特異度97%,33研究から解析した後者の感度は65%,特異度93%と報告されている11)。MRI拡散強調画像はFDG-PET/CTと同等の診断精度が示されており12)〜14),FDG-PET/CTが利用できない場合,MRIを行うよう提案する。

  • c.前述の画像検査では少なからず偽陽性の結果が存在するため,これらの画像検査の結果だけで治療方針を決定するには,その正確性は不十分である。非小細胞肺癌の縦隔病期診断における超音波気管支鏡ガイド下針生検(endobronchial ultrasound-guided transbronchial needle aspiration;EBUS-TBNA)の診断精度を調べた9研究1,066症例のメタアナリシスによると,診断感度は90%,特異度は99%と報告されている15)。非小細胞癌の縦隔病期診断における経食道的超音波内視鏡ガイド下針生検(endoscopic ultrasound-guided fine-needle aspiration;EUS-FNA)の診断精度を調べた18研究1,201症例のメタアナリシスによると,感度は90%,特異度は97%と報告されている16)。その他のメタアナリシスにおいても,EBUS-TBNA,EUS-FNAとも感度は概ね90%,特異度100%というデータで一致している17)〜19)。合併症は稀であるが,大出血や縦隔炎など重篤な合併症も報告されており注意を要する20)。EBUS-TBNAとEUS-FNAは,両方行ったほうがそれぞれ単独に比べ正確性が上がる19)21)22)ため,必要と判断される症例に対してはEBUS-TBNAとEUS-FNAを両方行うよう提案する。多施設共同ランダム化比較試験において,画像検査で縦隔リンパ節転移が疑われる症例に対し,縦隔鏡検査の前に超音波内視鏡検査を加えることにより,合併症の頻度を上げることなく診断感度が上がり,不要な肺切除を減らすことが報告されており23),画像検査で縦隔リンパ節転移を疑う場合,超音波内視鏡検査による病理学的診断を推奨する。なお,超音波内視鏡(EBUS-TBNA,EUS-FNA)を用いたN因子診断においては,画像診断に基づいた選択的生検よりも,系統的な生検を実施することが提案される24)。画像検査で縦隔リンパ節転移を疑わない症例に対しては,PET/CTの陰性的中率は高いことから,すべての症例に生検を行うように勧めるだけの根拠は明確ではないが,PET/CTでN2偽陰性の予測因子となる原発巣が3cm以上の症例25),原発巣が中枢に存在する症例26),画像上N1が疑われる症例27)に対しては,画像上縦隔リンパ節転移を疑わなくとも超音波内視鏡検査により生検を行うことを考慮してもよい。

  • d.縦隔鏡検査とEBUS-TBNAの非小細胞肺癌の縦隔病期診断に対する効果を比較したメタアナリシスによると縦隔鏡の感度は86%,特異度100%とされ,EBUS-TBNAの感度84%,特異度100%と差はなく,合併症の頻度はEBUS-TBNAのほうが少ないことが示された18)。超音波内視鏡検査後の縦隔鏡検査の必要性に関しては議論があるものの28)29),超音波内視鏡検査陰性症例に縦隔鏡検査を加えると,縦隔リンパ節転移を診断できる症例が増えることが報告されており23)27),術前の画像検査で縦隔リンパ節転移が疑われ,超音波内視鏡検査では縦隔リンパ節転移を認めなかった場合,必要と判断される症例に対して,縦隔鏡検査などの外科的生検を行うよう提案する。

CQ22.

M因子診断のために,必要な検査は何か?

  • FDG-PET/CTで,単発の遠隔転移が疑われた場合は,可能なかぎり他の画像診断や病理学的診断で転移であることを確認するよう提案する。

解 説
  • a.遠隔転移の検索において,FDG-PET/CTは造影CTに比べ感度・特異度が高く,FDG-PET/CTによる遠隔転移診断の感度が77〜100%,特異度が93〜100%との報告や30)〜33),肝,副腎での陰性的中率が95%,骨では90%との報告がある34)35)

     脳転移の検索において,造影MRIは造影CTや単純CT,MRIに比べて高い感度を示し,腫瘍径の小さな転移を多く検出すると報告されている36)37)が,体内金属などにより造影MRI撮影できない場合は,造影CTでも代用可能である。脳転移検索におけるFDG-PET/CTについては18FDGは正常脳組織への集積がみられるために感度は24〜27%38)39)と低く,勧められない。

     ただし,原発巣が2cm以下のGGNで充実成分の比率が25%以下の症例は遠隔転移がほとんどないことが報告されており40),このような症例は確定診断を待たずに手術を施行する症例も多く,FDG-PET/CT,頭部造影MRIもしくはCTなどによる病期診断は必須ではない。

     コントロール不良の糖尿病,閉所恐怖症,自施設および近隣の施設にPET/CTの設備がない等,FDG-PET/CTが施行し得ない場合,造影CTはFDG-PET/CTに比べると転移巣検索の感度・特異度は劣るものの,転移巣は一般的に血流が豊富で造影CTでの造影効果が高いため,肝,副腎,腎などへの転移検索に造影CTは有用である41)

     また,骨転移の検索に関して,複数のメタアナリシスで骨シンチグラフィの感度82〜86%,特異度62〜88%と報告されており42)43),FDG-PET/CTの感度92%,特異度98%に比べるとやや劣るものの,Schirrmeisterらの報告では無症候の肺癌患者に骨シンチグラフィを施行しなかった場合,14〜22%の患者が骨転移を見逃されるとされ44),FDG-PET/CTが施行し得ない場合には骨シンチグラフィを施行するように勧められる。しかし,外傷や変形性関節症などでは偽陽性となり得るため注意が必要である。

  • b.遠隔転移の検索にFDG-PET/CTは有用であるが,感染性疾患,肉芽腫性疾患,副腎腺腫などで偽陽性となることが指摘されており45)46),注意が必要である。

     特に副腎は肺癌の転移先として最も多い臓器の1つであるが47),副腎腺腫でもFDG集積がみられるため鑑別は重要である。CTで一側性の副腎腫大がみられた場合に副腎転移の可能性が高いとする報告もあるが30),有意でないとの報告もあり48),それだけで良悪性の判断はできず,MRIの脂肪抑制画像やダイナミックMRIなどの検査を組み合わせたり,経皮針生検などが有用であることが指摘されている47)〜51)。このように単発の遠隔転移が疑われた場合は,可能なかぎり他の画像診断や病理学的診断で転移であることを確認することが勧められる。
